1. How to file Maturity Claim?

Claim Intimation
Bajaj Allianz would send intimation letter to the customer 1 month earlier to the maturity date along with maturity claim discharge form & annuity form (if pension policy).
 
Required Documents
  Maturity Discharge Form.
  Original policy document.
  Photocopy of Life assured Standard KYC documents (e.g. Identity Proof and Address Proof).
  Photocopy of Life assured Bank pass book or Original cancelled cheque containing IFSC code.
  Duly filled ECS mandate.
  If Life assured name in bank pass book is not matching with other documents, then NEFT Form (sealed and signed by bank manger) should be submitted.
 
Additional document for Annuity Plan
  Annuity Form(in case of Pension Policy).
 
Settlement of Claim
  On compliance of all the above documents, the maturity claim amount shall be paid.
  For Pension Policy, the amount directed by customer towards purchase of Annuity would be sanctioned in favor of the Annuity provider (the insurer from whom the Immediate Annuity would be bought) and remaining amount would be credited to the policyholder's account.
 

2. How to file Death Claim?

Claim Intimation
Visit or notify our nearby branch or H.O. with a copy of Death Certificate regarding the death of Life assured and get the intimation letter through mail/courier.
 
Required Documents
  Duly filled claim Intimation letter from claimant.
  Death Certificate (original or attested).
  Policy Bond / Indemnity bond (If the bond is lost).
  Photocopy of Life Assured ID proof & address proof.
  Photocopy of claimant ID proof & address proof.
  Photocopy of claimant Bank pass book or Original cancelled cheque containing IFSC code.
  Duly filled ECS mandate.
  If claimant name in bank pass book is not matching with other documents, then NEFT Form (sealed and signed by bank manager) should be submitted.
  Legal heir certificate (Required if claimant is other than nominee. Legal heir certificate should be issued by Tahsildar or by court)
  Affidavit from notary (In case of name of Life assured or Life assured father or claimant is mismatching with submitted documents).
  Upon which barcode claim forms will be generated, which needs to be filled and submitted.
 
Additional documents for ACCIDENT / SUICIDE / MURDER
  Along with the above documents few additional documents are required.
  FIR
  Post Mortem report.
  Driving license of Life Assured.
  Chemical Analysis / Viscera Report.
  News Paper cutting.
  Police Inquest report (PIR).
  Final verdict from judicial magistrate.
 
Settlement of Claim
  On receiving of all the above necessary documents, the claim settlement process will start within seven working days.
 

3. How to file Rider Claim?

RIDER CLAIM:Two major types of rider claims.
 
Comprehensive accidental protection:contain below three benefits.
 
  Accidental Death Benefit (ADB).
  Permanent Total/Partial Disability Benefit (PTPDB)
 
Critical Illness Rider: For CI benefit, there is a waiting period of 6 months (180 days) form the date of risk. Only for the given 11 critical illness the claim will be considered.
 
  First Heart Attack.
  Coronary Artery Disease Requiring Surgery.
  Stroke
  Cancer
  Kidney Failure
  Major Organ Transplantation
  Multiple Sclerosis.
  Aorta Graft Surgery.
  Primary Pulmonary Arterial Hypertension
  Alzheimer’s disease
  Paralysis
 
Note:
 
Disability benefit will be paid depending on the policy terms and conditions.
 
Claim Intimation
 
Visit or Notify our nearby branch or H.O. about the reason for rider claim and get the intimation letter through mail/courier.
 
Required Documents
 
  Claim intimation letter.
  Hospital Discharge / Summary sheet (Original).
  Photocopy of claimant Bank pass book or original cancelled cheque containing IFSC code.
  Duly filled ECS mandate.
  If claimant name in bank pass book is not matching with other documents, then NEFT Form (sealed and signed by bank manager) has to be submitted.
  Photo copy of Life assured ID proof & address proof.
  Affidavit from notary (In case of name of Life assured or Life assured father is mismatching with submitted documents).
  All original bills, ECG, X-rays.
  Upon which barcode claim forms will be generated which needs to be filled and submitted.
 
Additional documents for Permanent or partial disability rider claim
 
  Along with the above documents few additional documents are required.
  Photograph of disabled organ of Life assured. 
  FIR copy
  Driving license of Life assured
 
Settlement of Claim
 
  On receiving of all the above mandatory documents the claim settlement process will start within seven working days.
 
Note:
 
  Claimant Bank pass book photocopy and All KYC documents (Photocopy of ID and Address Proof) have to have self attestation of claimant.
  Claimant means nominee; if nominee is also deceased then Legal heir will be the claimant.
  Read your policy document carefully.
  Verify all the details featuring on your policy document and inform us immediately in case of any discrepancies.
  Do not keep life insurance policies in your safe deposit box. In most states, safe deposit boxes are sealed temporarily upon the death of the owner, which could delay claim process.
  Do not forget to note down the policy number.
