Policy Servicing

1. How to file request pertaining to Address correction/Address Change?

Intimation
Visit or notify our nearby branch or H.O. with Policy number and Policy holder date of birth. Upon barcode forms will be generated, barcode forms to be duly filled and submitted to concern department with required documents.
 
Required Documents
  Duly filled barcode forms.
  Copy of Policy Bond (first 3pages only).
  Copy of Application form.
  Copy of ID proof.
  Copy of Address proof.
  Duly filled authorization form.
 
Request Fulfilment
Once all the documents are received, your request will be fulfilled within 7 working days and an acknowledgement letter will be sent to the Policy holder.
 
Note:
Address Correction
Correction in the address will be done, if there is any variation in the printed policy document and address proof given at the time of login.
 
Address Change
Change in the address will be done, if the policy holder has shifted his residence or if the complete address is printed wrongly in the policy document.

2. How to file request pertaining to Nominee Correction/Nominee Change?

Intimation
Visit or notify our nearby branch or H.O. with Policy number and Policy holder date of birth. Upon barcode forms will be generated, barcode forms to be duly filled and submitted to concern department with required documents.
 
Required Documents
  Duly filled barcode forms.
  Copy of Policy Bond (first 3pages only).
  Copy of Application form.
  Copy of policy holder and nominee ID proof.
  Copy of policy holder and nominee Address proof.
  Duly filled Authorization form.
  Appointment of Appointee [ If Nominee is Minor ].
 
Request Fulfilment
Once all the documents are received, your request will be fulfilled within 7 working days and an acknowledgement letter will be sent to the Policy holder.
 
Note:
Nominee Correction
Correction in the nominee name will be done, if there is any difference of name printed in policy document and in the application form.
 
Nominee Change
Change in the Nominee can be done, if the policy holder is willing to change nominee or if the complete Nominee name is wrongly printed in the policy document.

3. How to file request pertaining to Date of Birth (DOB) correction?

Intimation
Visit or notify our nearby branch or H.O. with Policy number and Policy holder date of birth. Upon barcode forms will be generated, barcode forms to be duly filled and submitted to concern department with required documents.
 
Required Documents
  Duly filled barcode forms.
  Copy of Policy Bond (first 3pages only).
  Copy of Application form.
  Copy of policy holder ID proof.
  Copy of policy holder Address proof.
  Duly filled Authorization form.
 
Request Fulfilment
Once all the documents are received, your request will be fulfilled within 7 working days and an acknowledgement letter will be sent to the Policy holder.
 
Note:
If Policy is issued with non standard age proof, request for DOB correction will not be accepted.

4. How to file request pertaining to Fund switching?

Intimation
Visit or notify our nearby branch or H.O. with Policy number and Policy holder date of birth. Upon barcode forms will be generated, barcode forms to be duly filled and submitted to concern department with required documents.
 
Required Documents
  Duly filled barcode forms.
  Copy of Policy Bond (first 3pages only).
  Copy of Application form.
  Copy of policy holder ID proof.
  Copy of policy holder Address proof.
  Duly filled Authorization form.
 
Request Fulfilment
Once all the documents are received, your request will be fulfilled within 10 working days and an acknowledgement letter will be sent to the Policy holder.

5. How to file request pertaining to Mode of payment change?

In case customer wants to change the mode of payment from half yearly to yearly mode, below mentioned document should be submitted.
 
Required Documents
  Request letter from policy holder addressing to BAJAJ ALLIANZ.
  Copy of Policy Bond (first 3pages only).
  Copy of Application form.
  Copy of policy holder ID proof.
  Copy of policy holder Address proof.
  Duly filled Authorization form.
Request Fulfilment
Once all the documents are received, your request will be fulfilled within 15 working days and an acknowledgement letter will be sent to the Policy holder.
 
Note:
Mode of payment can be changed only at policy anniversary.

6. How to file request pertaining to Assignment of Policy?

Intimation
Visit or notify our nearby branch or H.O. with Policy number and Policy holder date of birth. Upon barcode forms will be generated, barcode forms to be duly filled and submitted to concern department with required documents.
 
Required Documents
  Duly filled barcode forms.
  Original Policy Bond.
  Request letter addressing to BAJAJ ALLIANZ (From both the Parties).
  KYC forms from both the parties.
  Copy of assignee and assignor ID proof
  Copy of assignee and assignor address proof.
  Duly filled Authorization form.
 
Request Fulfilment
Once all the documents are received, your request will be fulfilled within 20 working days and an acknowledgement letter will be sent to the Policy holder.

7. How to file request pertaining to Indemnity bond?

Required Documents
  Request letter from policy holder addressing to BAJAJ ALLIANZ.
  State wise stamp paper (Link).
  DD of Rs. 300 in the name of Bajaj Allianz life insurance Company limited.
  Copy of policy holder ID Proof (with self attestation of policy holder along with notary seal and signature).
  copy of policy holder Address proof (with self attestation of policy holder along with notary seal and signature).
  Copy of witness ID Proof (with self attestation of Witness along with notary seal and signature).
  Copy of witness Address Proof (with self attestation of Witness along with notary seal and signature).
 
Request Fulfilment
Once all the documents are received, your request will be fulfilled within 10 working days and an acknowledgement letter will be sent to the Policy holder.

8. How to file request pertaining to free look cancellation?

Required Documents
  Request letter from policy holder addressing to BAJAJ ALLIANZ.
  Original Policy Documents
  Copy of policy holder ID proof.
  Copy of policy holder Address proof.
    Proof of policy document receipt (courier / Reg post).
  Duly filled EPS form (IFSC or MICR code has to be mentioned correctly) or cancelled cheque (If the cheque contain policy holder name).
 
Request Fulfilment
Once all the documents are received, your request will be fulfilled within 30 working days.
 
Note:
Please read the document carefully, once you receive the document before going for free look cancellation.

9. How to file request pertaining to policy surrender?

There are two types of Surrender
  1. Full Surrender
  2. Part withdrawal
 
Intimation
Visit or notify our nearby branch or H.O. with Policy number and Policy holder date of birth. Upon barcode forms will be generated, barcode forms to be duly filled and submitted to concern department with required documents.
 
Required Documents
 
Full Surrender
  Duly filled barcode forms.
  Request letter from policy holder addressing to BAJAJ ALLIANZ.
  Original Policy Bond
  Duly filled Authorization form
  Copy of policy holder ID Proof.
  Copy of policy holder Address Proof.
  Specimen Signature Form.
  Duly filled EPS form (IFSC or MICR code has to be mentioned correctly) or cancelled cheque (If the cheque contain policy holder name).
 
Partial Withdrawal
  Duly filled barcode forms.
  Request letter from policy holder addressing to BAJAJ ALLIANZ.
  Copy of policy bond (first 3pages only).
  Duly filled Authorization form
  Copy of policy holder ID Proof.
  Copy of policy holder Address Proof.
  Specimen Signature Form.
  Duly filled EPS form (IFSC or MICR code has to be mentioned correctly) or cancelled cheque (If the cheque contain policy holder name).
 
 
Request Fulfilment
Once all the documents are received, your request will be fulfilled within 20 working days.
 
Note:
 
Some % amount should be retained in the fund value. % will vary with the product.
 

10. How to file request pertaining to correction in refund cheque?

 
Required Documents
1. Name correction in the cheque
  Request letter from policy holder addressing to BAJAJ ALLIANZ.
  Original Refund Cheque.
  Proof stating Correct Name.
  Copy of proposer ID Proof.
  Copy of proposer Address Proof.
  Duly filled EPS form (IFSC or MICR code has to be mentioned correctly) or cancelled cheque (If the cheque contain policy holder name).
 
2. Cheque Revalidation
  Request letter from policy holder addressing to BAJAJ ALLIANZ.
  Original Refund Cheque.
  Copy of proposer ID Proof.
  Copy of proposer Address Proof.
  Duly filled EPS form (IFSC or MICR code has to be mentioned correctly) or cancelled cheque (If the cheque contain policy holder name).
 
3. Stop payments and issuance of fresh cheque
  Request letter from policy holder addressing to BAJAJ ALLIANZ.
  Copy of proposer ID Proof.
  Copy of proposer Address Proof.
  Duly filled EPS form (IFSC or MICR code has to be mentioned correctly) or cancelled cheque (If the cheque contain policy holder name).
 
Note:
Stop payment for the refund cheque can be done, if the refund cheque is lost.
4. Refund cheque to be issued in Nominee name [ in case of proposer death]
  Request letter from policy holder addressing to BAJAJ ALLIANZ.
  Legal hire certificate on 100 Rs bond paper.
  Copy of nominee ID Proof.
  Copy of nominee Address Proof.
  Death Certificate.
  Duly filled EPS form (IFSC or MICR code has to be mentioned correctly) or cancelled cheque (If the cheque contain policy holder name).
 
Note:
Notary attestation should be present on all documents.
 
Request Fulfilment
Once all the documents are received, your request will be fulfilled within 30 working days.
 
Note:
  Read your policy document carefully.
  Applicant has to do his/her signature on all the documents while filing request (Signature should be in blue ink only).
  Policy holder/proposer signature should not mismatch with application and other document submitted.
